Bridges to Housing in Our Communities

Multnomah County

Human Solutions Human Solutions is currently serving fifty (50) families throughout Portland, Oregon. Human Solutions offers intensive case management coupled with a Family Team approach to coordinate all other case managers and social service providers. Together the families and providers will develop an Action Plan that will be used as a tool to clarify goals and steps towards reaching them, with the overarching goals of income self-sufficiency and permanent housing stability.

Human Solutions has developed strong partnerships with mental health providers and other community resources.  Strong resident services on site or near each address are also a component of the project.  Child care subsidy will be available through the Children’s Investment Fund. 

Human Solutions is currently serving Bridges to Housing families at Arbor Glen, Humboldt Gardens and several scattered site properties in east Portland. At Humboldt Gardens, Human Solutions has partnered with the Housing Authority of Portland (HAP), and families at this site will also benefit from Family Self Sufficiency Programming at Humboldt Gardens.

Central City Concern – Family Alcohol and Drug Free Housing Network (FAN). Fifteen (15) Bridges to Housing families were incorporated into Central City Concern's (CCC) existing Family Alcohol and Drug Free Housing Network and housing in a variety of family housing properties. This program is an effort to improve coordination and a continuum of care to Alcohol and Drug Free Community (ADFC) families across multiple systems. CCC partners with other treatment and social service providers to provide coordinated services to families.

Impact Northwest
Impact Northwest has been working with homeless families for the past twenty years. They have experience working with families with disabilities, and multiple barriers such as domestic violence, criminal history, addiction, mental illness, immigration, and developmental disabilities.

Impact Northwest has partnered with several housing providers to house Bridges to Housing families:

  • Hacienda CDC andImpact Northwest have partnered together to provide Bridges to Housing to eight (8) families at Hacienda’s two Clara Vista apartment complexes.
  • Catholic Charities and Impact Northwest have partnered together to provide housing at three sites. Catholic Charities will house two (2) Bridges to Housing families at Kateri Park and four (4) families at Howard House.At Howard House, the target population for this is high resource using homeless families with at least one child under the age of 18 and at least one member of the family with a disability Most recently, Catholic Charities also began housing fourteen (14) Bridges to Housing families at their new development, Esperanza Court.
  • Rose CDC and Impact Northwest have partnered together to provide Bridges to Housing to eleven (11) families at Rose CDC's Leander Court. Leander Court includes many family amenities, including large two, three and four bedroom units, playground areas, and an on-site daycare facility. Five (5) Bridges to Housing families are also being housed at several scattered site properties owned by Rose CDC.
  • Innovative Housing, Inc and Impact Northwest have partnered together to provide housing to fifteen (15) families at a new development, Broadway Vantage. Broadway Vantage. This new development is one block from the 82nd Avenue MAX station and includes a community room and playground.

El Programa Hispano
El Programa Hispano is the newest Bridges to Housing service provider, and began serving families in summer 2009. Since 1982, El Programa Hispano has provided a wide array of social services to low-income Latinos in the Portland metro area. El Programa Hispano is partnering with Hacienda CDC to serve five (5) Bridges to Housing families at the Miraflores housing project.
